South Hardwick Primary Care Network (PCN) is looking for a Community GP to support their multi-disciplinary team providing healthcare to housebound people and those in care or residential facilities across the PCN footprint.
The PCN is also looking for Advanced Clinical Practitioners (ACPs) to join at the start of a new healthcare initiative developing their Acute Home Visiting Service.
